Management of thyroid ophthalmopathy: the approach and challenges

چکیده
Thyroid ophthalmopathy, or Graves’ ophthalmopathy, is the most common cause of exophthalmos in adults. Graves first described thyroid disease with associated ophthalmopathy in 1835. To date, the pathogenesis of thyroid ophthalmopathy remains speculative, as reflected by the numerous names by which it is described such as dysthyroid ophthalmopathy or thyroid eye disease, thyroid orbitopathy, euthyroid ophthalmopathy, euthyroid Graves’ disease, thyrotoxic exophthalmos, infiltrative ophthalmopathy, or malignant exophthalmos. It is considered to be an organspecific autoimmune disease, strongly associated with Graves’ hyperthyroidism. For some patients, thyroid function may remain normal or subnormal for part or all of the course of the disease. Some evidence indicates that radioiodine ablation, surgical thyroidectomy, or hypothyroidism after medical treatment can initiate or exacerbate thyroid eye disease. Smoking has also been shown to be a risk factor for developing more severe ophthalmopathy.

Management of patients with thyroid ophthalmopathy
Patients with thyroid-associated ophthalmopathy (TAO or thyroid eye disease TED) may present diagnostic and therapeutic challenges. As the disease evolves, a myriad of ophthalmic complications may occur that requires the involvement of multiple ophthalmic disciplines. Orthoptists are valuable members of the eye care team who are often called upon to manage the diplopic complaints of these patie...

CASE REPORT The effect of a somatostatin analogue in a patient with Graves' ophthalmopathy is presented, including data on the dose requirements and the results of therapy. DISCUSSION There are few effective options for the management of Graves' ophthalmopathy, a cell-mediated immune co-morbidity of thyroid disease. Somatostatin analogues inhibit lymphocyte proliferation and activation, and a...

Thyroid associated ophthalmopathy is a constellation of symptoms caused by an autoimmune process involving the orbital tissue. It is common in hyperthyroid patients due to Graves' disease and also reported in euthyroid and hypothyroid Graves' patients with positive thyroid receptor antibodies. But in Hashimoto's thyroiditis, thyroid associated ophthalmopathy is a rarely reported and poorly unde...
متن کاملSCIENTIFIC REPORT The relation of Graves’ ophthalmopathy to circulating thyroid hormone status
Aim: The risk factors and epidemiological data for Graves’ ophthalmopathy with and without abnormal circulating thyroid levels were examined to determine the relation of thyroid dysfunction to ophthalmopathy. Methods: The authors retrospectively evaluated 482 patients seen with Graves’ ophthalmopathy. Of these, 413 were classified as having abnormal levels of circulating thyroid hormone (ALTH) ...
